NJR

Meeting with the Manufacturers

May 15, 2025

π-RIPI (Creation of the prototype of the National Registry of Implantable Prostheses (RIPI) data collection platform) is a project started within the Spoke 2 (Technology transfer, new entrepreneurship, business incubation and acceleration) of the NextGenerationEU project Rome Technopole and is carried on in cooperation with BVTech and Confindustria-DM, both partners of Rome Technopole and coordinators of the Flagship Project FP4 (Development, innovation and certification of medical and non-medical devices for health).

The RIPI registry was established at the Istituto Superiore di Sanità in 2017 (DPCM 3/3/2017) to monitor, at the national level, the safety of the implanted prostheses and to ensure patient quick traceability in case of adverse event (recall). It is conceived as an umbrella covering a set of registries each referred to a specific class of medical devices. The Italian Arthroplasty Registry (RIAP) is one of the registries considered by RIPI to monitor safety and effectiveness of joint prostheses. In 2021, ISS signed an agreement with the National Joint Registry (NJR) to be allowed to access the NJR Component Classification and Database.

The objective of π-RIPI is to design the platform supporting the RIPI data collection and to implement the module relevant to the identification and characterization of the joint prostheses and to interconnect it with other existing international databases.

On May 15 and 16, 2025 a meeting was held at ISS aimed to set up and agree on the operational requirements needed to ensure the interconnection and interoperability of π-RIPI with the NJR Component Database. One session was dedicated to meet the Industry.

The programme of this session and the presentations are available here.

The title of the symposium is a question whose answer is certain: the use of the information provided by arthroplasty registries can improve the surgeon performance.

Since the Seventies, in Scandinavian countries data from arthroplasty registries have been the main source of information for surgeons and over the years have led to standardized practice and reproducible
results. In the United Kingdom, based on a greater number of registered procedures and a highly refined methodology, the National Joint Registry (NJR) has achieved a better understanding of the variables that can affect joint replacement outcomes. In addition, the dissemination of the data collected and the results reached by registries to the Orthopedic community and the public has led to a shared knowledge capable of addressing medical practice and clinical research.

Today RIAP is firmly established in many Italian regions. As a matter of fact, by law it is now mandatory for orthopedic surgeons to provide the regional registries on a regular basis and that the companies and bodies of the Italian National Health Service must keep them continuously updated.

What is left to verify is RIAP’s capacity of improving the Italian clinical practice of orthopedic surgeons.

We discussed the issue, comparing some experiences and health care systems with Timothy Wilton (NJR Medical Director), Keith Tucker (Chair of ODEP), Gustavo Zanoli and Emilio Romanini (SIOT-GLOBE) and Marina Torre (RIAP Director RIAP, Italian National Institute of Health).

International Strategic workshop Tools to identify and characterize implantable devices: the perspective of the RIAP-NJR collaboration 1st March 2018.

The Italian Arthroplasty Registry (Registro Italiano ArtroProtesi, RIAP), funded by the Italian Ministry of Health, in 2012 in collaboration with Assobiomedica set up the RIAP-DM Dictionary. Since then, manufacturers have continuously updated this database with the information essential to identify and track the implants. Quality of the collected information is checked by comparing it with data available from the Ministry of Health
National medical device database (BD/RDM).
The National Joint Registry for England, Wales, Northern Ireland and the Isle of Man (NJR) has recently upgraded and developed its existing component database, in collaboration with the EndoProthesen Register Deutschland (EPRD), to provide a structured database directly fed by manufacturers that includes both the information necessary to track and identify the implanted device and to describe its technical characteristics, these features being an indispensable requirement to compare the performance of different prostheses.
The future perspective of setting up a collaboration between NJR and RIAP is to link the RIAP-DM Dictionary with the NJR-Component Library in order to create a single international database of the orthopedic prostheses implanted in United Kingdom, Germany and Italy. After the potential establishment of such a partnership, manufacturers would be requested to provide the same information only once and all the parties involved in the arthroplasty registries data collection would benefit from the use of a shared and internationally verified component database.

Aim of the Workshop is to present the architecture and organization of the NJR Component Library in the perspective of the establishment of a larger database that could also consider the Italian Arthroplasty Registry and to show the Italian medical device manufacturers and distributors how the Library is fed.
